New Patients

ENT & Plastic Surgery Specialists of Louisiana welcomes new patients of all ages. Below are a few things that you, as a new patient, will need to do prior to or at the time you first receive care from our physicians:

  • Review your insurance coverage for office visits and any anticipated procedures.
  • Complete our New Patient Forms
  • Bring your driver’s license and your primary or secondary insurance cards.
  • Bring a list of your current prescription medications, OTC medications, or supplements.

Returning Patients

When you return to our office, it is important that you always:

  • Bring your current insurance card(s).
  • Current driver’s license to make the registration process easier.
  • Update us regarding any changes of address or phone number.
  • Update us regarding any changes in medications, OTC medications, or supplements you take.

Office Hours

Our offices are open between the hours of 8:00 a.m. and 5:00 p.m. Monday through Friday (excluding holidays) and closed on the weekends.

Emergencies

If you have a medical emergency, please call 9-1-1. For non-emergent matters outside of our normal business hours, the on-call physician can be reached by calling our office at 985-845-2677. Your call will roll over to the answering service after hours.
